Community Care Center is a locked psychiatric facility in Duarte, California. The Special Treatment Program is certified by DHCS – Department of Healthcare Services and licensed by the Department of Public Health. We offer specialized care and treatment for individuals 18 and older with short and long-term psychiatric conditions.

Our residents will have a clinical care team that includes the patient’s psychiatrist, primary care physician, nursing, psychologist, and a psychological counselor. This multidisciplinary team develops a person-centered care plan design to help individuals modify their behaviors and learn skills that will help improve their quality of life. Specifically, these plans focus on strategies to reduce problematic behaviors through meaningful activities.

Community Care Center also offers behavioral therapy that is designed to minimize safety risks while preserving a comfortable environment. Policies and procedures also focus on resident care and safety.

Activity Program

A positive and uplifting atmosphere can set the tone for a resident’s stay. Community Care Center employs outgoing and friendly professional staff to create social programs and activities. We often specialize in activities to match the capabilities and needs of each resident. This also has a powerful impact on the healing process.

Active resident and family councils, Volunteer programs, Musical performances, Current event updates, Reading the newspaper, Singing exercises, Fitness activities, Stretching exercises, Religious meetings, Entertainment, Games, Trivia

How To Join The Fun:

We provide each resident with a monthly activity calendar to keep them informed of upcoming events such as birthday parties, movies, musical programs, outings, and other special events.

Psycho-Educational Groups

Coping with mental illness – Ways to reduce stress and problem solving

Social Skills Training

Focus on symptom management – Basic communication skills – Recreation for leisure – Community reintegration

Anger Management

From the biological, emotional, behavioral, and cognitive perspective

Medication Management

Our psychiatrist will see clients a minimum of twice per month. Psychotropic PRN’s are used only when clinically indicated.

Applied Behavioral Analysis

A focused effort is used to breakdown behaviors and interventions into their most basic steps.

Behavioral Treatment

The key to changing behavior is understanding the client’s behavioral history and its functional manifestations.

Vocational Rehabilitation

When a client assessment indicates a benefit from vocational services, this will become part of the client’s individual treatment plan

Substance Abuse

Clients who have an identified need will attend community-based substance abuse counseling three times per week.

Educational Services

Duarte schools provide on-site education to clients who qualify for these services.

Guidelines for Family and Friends

Visiting hours are from 10:00 AM – 2:00 PM Monday – Saturday. Each visit is 1/2 hour in length. You must call the Counseling office or SSD in the Bungalows to schedule a time. No more than 2 visitors per resident per visit. While visiting, all visitors are expected to abide by all of the rules/policies of the facility. Please review our guidelines prior to your visit with us to ensure a smooth and productive visit with your family or friend.
